MD Anderson registered this dose-escalation study in 2010 and began dosing in 2012, planning three participants per dose level with pharmacokinetic sampling on days 1 and 22. It was terminated at the principal investigator's request after four participants and no safety, pharmacokinetic or efficacy results were ever posted, so no human dose has been established.

The insulinogenic index improved by 48.5%, food intake fell, and lean monkeys given the same dose did not lose weight.

The author reports the number he sees used while stating plainly that no validated human dose exists and that anyone administering it is outside any tested safety framework. He recommends baseline and serial creatinine, BUN, eGFR and urinalysis for anyone who proceeds, and his page contains some descriptive errors about the original papers.

How a course is commonly described

  1. Weeks 1-2Community-reported

Community pages describe a low flat starting dose, around 250 mcg daily, with abdominal measurements as the first thing to change. In the primate dose-finding work the first creatinine rises appeared once the dose passed 0.25 mg/kg, which is the point the animal data flag rather than any body measurement.

  2. Weeks 2-4Preclinical

The 28-day block is the only duration any published protocol used, in mice, in three primate species and in the terminated human trial. In the fixed-dose monkey study weight, BMI and abdominal circumference fell steadily across these four weeks while food intake also dropped.

  3. Recovery, 4 weeksPreclinical

Every primate study paired 28 days of dosing with a 4-week recovery period. Creatinine and the urinary findings returned toward baseline during it, and weight, BMI and abdominal circumference started to reverse, which is the clearest sign that the effect depends on continued dosing.

  4. Repeat cyclesCommunity-reported

No source describes a tested repeat cycle. Community pages cap use at 4 to 8 weeks and one database notes that nothing is known about the effect of repeated rounds of adipose vascular ablation on kidney reserve, because the published exposure stops at one 28-day block.

  • No validated human dose, schedule or cycle length exists for adipotide. The flat milligram and microgram figures on community pages are not the per-kilogram doses the animal studies used, and the only registered human trial closed after four participants without posting results.
  • Kidney proximal tubule injury was the dose-limiting finding in every primate study, appearing above 0.25 mg/kg and resolving in the 4-week recovery periods. Prohibitin is also present in kidney tubule cells, which is the usual explanation offered for this off-target effect.
  • Published exposure stops at 28 days of daily dosing. The primate data also show that weight and waist measurements start to reverse once dosing stops, so nothing in the record supports a short course as a lasting change.

Overview

Adipotide, also written FTPP or prohibitin-TP01, is a two-part synthetic peptide designed at the University of Texas MD Anderson Cancer Center. A cyclic nine-residue homing sequence binds prohibitin on the lining of the blood vessels that supply white fat, and a D-amino-acid proapoptotic sequence then disrupts the mitochondria of those cells, so the fat depot loses its blood supply and is reabsorbed. The approach reversed diet-induced obesity in mice in 2004 and produced about 11% weight loss in obese rhesus macaques in 2011, alongside dose-dependent and reversible changes in kidney proximal tubule function. The only human trial, a phase 1 study in men with obesity and metastatic prostate cancer, was terminated after four participants without posting results; adipotide has no approval anywhere and is sold only as a research chemical.
